Perla Insurance Claims Guideline
Follow these steps and prepare the requirements below to support the immediate processing and settlement of your claim.
Guideline

Report the accident to the Claims Department or the nearest Perla Insurance Branch.

Submit the requirements needed for the immediate processing and settlement of the claim.
- 3
Have the unit inspected by an authorized Perla Insurance claims representative or accredited repair shop. Pictures will be taken during inspection.

Claim processing turnaround time: see Standard Turnaround Time below.
- 5
Bring the insured unit to an accredited non-CASA repair shop for repairs upon receipt of the Letter of Authority.
List of Requirements
- Insurance Policy Number / Plate Number of Insured Unit
- Photocopy of Registration Certificate and latest Official Receipt (OR)
- Photocopy of Driver's License and OR
- Police Report and/or Driver's Affidavit duly notarized
- Pictures of the damaged unit and repair estimate(s) (if available)
- Other documents that may be required if necessary
Includes Own Damage requirements, plus:
- Photocopy of Registration Certificate and latest Official Receipt (OR) of TP vehicle
- Photocopy of Driver's License and OR of TP vehicle
- Pictures of the damaged TP unit and repair estimate(s)
- Certificate of No Claim issued by TP vehicle insurer
- Other documents that may be required
Includes Own Damage requirements, plus:
- Original Medical Certificate
- Original Hospital Statement of Accounts / Charge Slips / Sales Invoice
- Doctor’s Prescription and Sales Invoice for Purchased Medicines/Apparatus Outside the Hospital
- Original Professional Fee Official Receipt
- Photocopy of Birth Certificate of the 3rd party & Parents' Marriage Contract (in case of death)
- Other documents that may be required
Same as Own Damage requirements, and report the incident to the nearest police station where the theft occurred.
For Carnap, additional requirements:
- Original Complaint Sheet and Alarm Sheet from Highway Patrol Group (HPG), Camp Crame (or Regional Office)
- Original Certificate of No Recovery from HPG, Camp Crame
- Two (2) or Three (3) sets of ignition keys, depending on the insured unit
- Bank certification stating the early payment amount (if mortgaged)
- Certified copy of Sales Invoice from the car dealer
Standard Turnaround Time (TAT)

Note: TAT starts 24 hours after completion of requirements.
